Member & Non-Member registration fees include daily breakfast, daily lunch and access to all CE presentations and exhibitors. Meeting badges and materials may be picked up at the Workshop registration desk at the Hyatt Grand Cypress.

Deadlines

March 31 - Early registration closes. Late fees apply to all registrations submitted after this date.

April 10 - Hotel room reservation deadline.

April 15 - Pre-registration closes. Registrations received after this date will be processed onsite.

Golf Outing

Cost: $80/person

Set in the midst of an open meadow, the New Course is Jack Nicklaus’ tribute and homage to the famed Old Course at
St. Andrews, Scotland. This Scottish links masterpiece is complete with double greens, stone bridges and walls, gorse
mounds, burns and pot bunkers as deep as 12 feet – all in the spirit of the true Scottish original. Very little water
comes into play on the New Course, but when it does, it will challenge you. Very few trees are located within the interior
of the course, creating generous landing areas. However, an errant tee shot could find its way into one of 150 pot
bunkers. Be sure to sign up for the golf outing when you register! Golf clubs are available to rent at $50.


Request for Claims Review Cases

The claims review sessions have been a highlight of previous Workshops. In order to be successful, educational, and informative, your enthusiastic participation is necessary. Please take the time to submit an interesting and/or questionable case to share with your colleagues. We continue to learn from each other through the Spring Workshop, therefore, your contribution is vital.

Please submit your claim for review with as much documentation (x-rays, charting, etc.) as you have available for review to Dr. Robert Sherman at rsherman@hawaiidentalservice.com. If you have any further questions or comments, call Dr. Sherman at 808-529-9213 or email him at the above address.

 


Continuing Education (CE) Units

CE attendance forms will be in each individual registrant’s packet on-site. The 2017 Workshop offers 16.25 Continuing Education Units. Seminar registrants receive one CE credit for each clock hour attended. It is the responsibility of the attendee to submit continuing education documentation to their respective state for verification. The American Association of Dental Consultants is an American Dental Association (ADA) CERP recognized Provider.

ADA CERP is a service of the American Dental Association to assist dental professionals in identifying quality providers of continuing dental education. ADA CERP does not approve or endorse individual sources or instructors, nor does it imply acceptance of credit hours by boards of dentistry.

The American Association of Dental Consultants designates this activity for continuing education credits. Concerns or complaints about a CE provider may be directed to the provider or to ADA CERP at www.ada.org/cerp.


Cancellations/Refunds/Changes 

All refund requests must be in writing. Full refund, less a $50 per person administrative fee, will be issued for cancellations received on or before Friday, April 21and a 50% refund, less the administrative fee, if received Saturday, April 22 through Friday, April 28. No refunds will be mad e for cancellations received thereafter. Refunds are processed within 30 days after the meeting.
